    All registered members and teams must complete these forms. Print it, sign it, and return it to the Event Coordinator or any event Director NLT June 2, 2024.

  • You will also need copies of Birth Certificate and Hunter Education Certification Card attached to the form.

  • The above 2 forms must be completed: Individual Registration Form and Team Information Form. Each individual fee is $45 and the Coach fee is $20.

    There are a couple of ways to submit payment:
  • 1. Pay by Check attached to the forms that are mailed to:

  • 2. Pay via PayPal by logging into your account, select "Send money to friends and family" to treasurer@mdyhec.org or by Clicking the Donate button below.

  • If paying with paypal you will need to add a $5 convenience fee for every $100 paid. Ex:If the total registration fees are $200 total due is $210.
